While we cannot guarantee you a favorable outcome, we will do our best to help you receive a fair and timely response to your problem. Please include all relevant information and claim numbers in your correspondence—such as:
- Your address, home phone number and cell phone number so that we can obtain any additional information from you that might be necessary
- Copies of any related documents or correspondence that you may have from the agency involved
The Privacy Act of 1974 (5 U.S.C. § 552a) requires that Members of Congress or their staff have written authorization before they can obtain information about an individual’s case. U.S. Department of Veterans Affairs
The Department of Veterans Affairs runs programs benefiting veterans and members of their families. It offers education opportunities and rehabilitation services and provides compensation payments for disabilities or death.
Social Security Administration (SSA)
The United States Social Security Administration is an independent agency of the U.S. federal government that administers Social Security, a social insurance program consisting of retirement, disability, and survivor benefits.

The Centers for Medicare & Medicaid Services is part of the Department of Health and Human Services (HHS) that that administers theMedicare program and works with state governments to administer Medicaid, the Children’s Health Insurance Program (CHIP), and health insurance portability standards.
